Financial Aid Forms


If you plan to attend HSU Fall 2018 and/or Spring 2019 and are being asked to submit forms for the 2018-2019 academic school year (Aid Year: 2019), YOU ARE NOT AT THE CORRECT SITE. Click here to be directed to the Financial Aid FORMS & DOCUMENTS webpage and make sure you click on Student Forms: Fall 2018 - Spring 2019. 

This site is only for students enrolled for Fall 2017 and/or Spring 2018 who need to submit forms for the 2017-2018 academic school year (Aid Year: 2018). Click on All eForms and submit the appropriate form/s. If the form you need is not available, please contact our office at 707.826.4321.


  1. Refrain from submitting forms multiple times! By submitting duplicate forms, you create a delay in service to your file as well as other student files.

  2. Once you submit an eForm, your To-Do-List will be updated within 3-5 business days to indicate your document has been received. However, your file will only be reviewed when all documents requested have been received. Counselors review files in the order they completed and the review process may take 3-5 weeks.

  3. Use the UPLOAD A DOCUMENT feature in eFORMS! If you have been requested to submit other documents such as a W-2, a Tax Return Transcript, or are submitting supporting documentation, please use the Upload a Document feature. Make sure to always include your HSU ID# on all submitted documents. You can also submit documents to our office via fax at 707.826.5360 or you can stop by in person Monday - Friday from 8am - 5pm.
