Financial Aid Forms



PLEASE NOTE: This site is only for students who need to submit forms for Fall 2017-Spring 2018 school year.

If you need to submit forms for the Fall 2018 - Spring 2019 academic school year, you are not at the correct site, please click HERE to access HSU Webforms for Financial Aid Forms. Make sure you click on Student Forms: Fall 2018 - Spring 2019.  



  1. Check for critical requested Financial Aid items! Log into myHumboldt and review My Checklists or your Student Center's To-Do-List.

  2. Only submit requested forms! By submitting unrequested forms, you create a delay in service to your file as well as other student files.
  3. Refrain from submitting forms multiple times! By submitting duplicate forms, you create a delay in service to your file as well as other student files.
  4. Once you submit an eForm, your To-Do-List will be updated within 3-5 business days to indicate your document has been received. However, your file will only be reviewed when all documents requested have been received. Counselors review files in the order they completed and the review process may take 3-5 weeks.

  5. Use the UPLOAD A DOCUMENT feature in eFORMS! If you have been requested to submit other documents such as a W-2, a Tax Return Transcript, or are submitting supporting documentation, please use the Upload a Document feature. Make sure to always include your HSU ID# on all submitted documents. You can also submit documents to our office via fax at 707.826.5360 or you can stop by in person Monday - Friday from 8am - 5pm.

  6. For all student forms, LOGIN to submit requested eFORMS.
