Official Grade Change Forms must be obtained by faculty or staff members directly from the Office of the Registrar. Employees requesting grade change forms in person will be asked to show their HSU staff or faculty ID card, before forms are issued.
Grade change form will NOT be issued to students, nor will they be accepted when delivered by students.
After the Official Grade Change Form is completed it must be signed by the class instructor, department chair and college dean. Grade change forms for grades changing from "I", "RP", "SP" and "RD" require ONLY class instructor's signature.
Fully completed and signed forms should be returned to the Office of the Registrar by campus mail or in person (faculty or staff member with an HSU ID).
