Routing Instructions: 
This form is used for requesting to establish a new campus fee or adjust an existing campus fee. Complete parts 1, 2 AND 3 of this form. After completion, route all required documents according to Routing Order on form. Once all signatures are obtained, route to the Student Financial Services Manager in SBS 257.

In accordance with the provisions of the Chancellor's Executive Order 1102, the President is responsible for assuring that appropriate consultation occurs prior to adjusting any fee and before requesting the chancellor to establish a new fee. This form is used to facilitate this process.
