Routing Instructions: 
This form is used for requesting to establish a new campus fee or adjust an existing campus fee. Complete both pages of this form. After completion of both forms, route all required documents to Dean/Director and Vice President. Once all signatures are obtained, route to the Associate Vice President of Business Services (SBS 345).

In accordance with the provisions of the Chancellor's Executive Order 1054, the President is responsible for assuring that appropriate consultation occurs prior to adjusting any fee and before requesting the chancellor to establish a new fee. This form is used to facilitate this process.
