Outstanding Student Awards: Students Who Make a Difference 2014-2015

DEADLINE TO SUBMIT:  Friday, February 20, 2015
Nominations are now being accepted for Humboldt State University's annual Outstanding Student Awards. Faculty and staff are encouraged to nominate qualified candidates in the following categories.  Awards will be presented at our annual student recognition ceremony on Wednesday, April 8, 2015, 3:00-5:00 pm in the Kate Buchanan Room. Formal invitations will be sent to all nominees and their nominators. If you do not receive an invitation, please contact us at 826-3776.

PLEASE READ "NOMINATION PROCESS" and "AWARD CRITERIA" BELOW PRIOR TO STARTING THE ONLINE NOMINATION PROCESS.  All items listed should be submitted together. Your work will be saved after each page is completed, however if you leave the form part of the way through, please double check the completed pages for accuracy when you return to complete the nomination. Nomination Process

All materials must be submitted by midnight on Friday, February 20, 2015.  Late nominations cannot be accepted.  If additional information regarding the nomination process is needed, contact us at 826-3776.
Please submit complete nomination materials as follows:
1.     Fill in nominee's and nominator's Information in fields below;
2.     Attach nominee resume or letter :
-List those achievements, activities, awards, and other accomplishments attained while a student at HSU that relate to the nomination.
 -List other achievements, activities, awards and accomplishments attained while a student at HSU that are in addition to those mentioned above. (i.e. leadership positions, academic achievements, publications, committees, student government, club or organization involvement, community involvement, sports and recreation, etc.)
3.     Attach letter of support from the nominator(s) addressed to "Outstanding Student Awards Committee" describing the nominee’s achievements;
4.     Attach other appropriate materials for the committee’s review (i.e. posters, reports, art, etc. which can help the committee understand the student’s accomplishments);
5.     Attach jpeg photo of nominee for slide show which will be shown at the awards ceremony.

Read Award Criteria

The Awards Committee will consider the following applicable criteria when making their selections:

  • Nominee must be a student in good standing with at least a 2.5 cumulative GPA.   Award for Excellence in an Academic Discipline: 3.0 GPA
  • Volunteer versus paid positions: less emphasis will be placed on paid positions except when superior performance can be demonstrated
  • Accomplishments attained while the nominee was a student at HSU will be given priority; some achievements at prior institutions may be considered for transfer students.
  • Evidence of direct involvement
  • Level of commitment over a sustained period of time
  • Demonstrable and/or significant contributions
  • Inspiration to others
  • Innovation and creativity
  • Effectiveness
  • Enhanced and/or advanced the quality of student life
  • An individual may nominate only ONE student in each award category, except for the academic awards as follows.  Separate nomination forms and attachments must be submitted for each additional nomination:
    • Award for Excellence in an Academic Discipline:  An individual may nominate one student from each of the three academic Colleges.
    • Awards for Academic Excellence and Achievement:  An individual may nominate one student or team for each category.
  • A student, whose achievements clearly fit an award category other than that in which he or she was nominated, may be considered for a second category at the discretion of the Committee.
