Human Resources is pleased to announce our new online Appointment Document Form 104!
This new form will save time by electronically routing the form to all the people who need to approve it! You will be notified by email when the form is received in HR and when it has been approved.
This form is available online at www.humboldt.edu/forms/node/66
In order to help departments with the transition, paper documents will be accepted for a short time, however, we encourage you to begin using this electronic version.
For those of you who will be approving or reviewing appointment documents (Department Chairs/Lead Workers/Managers, Budget Analysts, Deans/Administrators):
Instead of paper forms for you to sign, you will now be receiving emails notifying that you have a 'Form 104' to approve. Use the link and instructions in the email to review and approve the document.
