Submission Information

Files must be less than 2 MB.

Vendor Information Required

The answers to the following questions will help us to identify any potential risk associated with the app from the vendor’s terms of use as stated in their end user license agreement and the level of accessibility and support they provide.

Please provide a link to the vendor’s End User License Agreement (EULA) and any other information governing the use of the app.
The application must meet HSU’s accessibility requirements. Please include documentation from the vendor to support their accessibility claims and ask them to provide a Vendor Product Accessibility Template and roadmap for the future.
